Just over a century has passed since man conquered the sky. Like any conquest, it requires a countless amount of risk-taking to achieve it; once achieved, the process of entrenchment begins, and with it, stabilisation. It is time and hard work that allow stabilisation to be assured. As has been the case with every human achievement, this process did not escape this chronological logic of growth. Of course, aviation is not outside the mechanics of the development of these achievements. Unfortunately, for this industry, the process of growth entails the loss of lives. More than a century after the conquest, it could be said that the activity is in the stabilisation phase, managing to achieve entrenched, reliable and sustainable safety standards. If it has reached this stage, it is the result of work, research, study and the construction of what we know today as safety culture. The conquest of the sky and safe activity require constant action to maintain and increase the levels of safety that have been achieved. This is why, in the present work, the most salient aspects of the development of concepts and techniques that serve, served and will serve crews to maintain and improve the aforementioned standards have been highlighted. As with all scientific and social evolution of mankind, great lessons have been learned through mistakes made. By virtue of this, some of the most serious aviation accidents were analysed so that through their conclusions, didactic and useful messages for the strengthening of operational safety remain. We would like to acknowledge all those whose efforts and knowledge have made it possible to learn lessons from these catastrophes. It is our intention that these pages can be useful in the daily flight activity and constitute a tool to raise awareness, help and strengthen safety.
